Registered Professional - Band 6 (Coventry)37.5 hours per week- we will consider part time hours for Caludon Centre, Coventry We have exciting opportunities for Registered Professionals who have 12 months + experience within Acute or Community setting of supporting Age Independent Adults with mental illness. The Urgent Care Access hub and Home Treatment Team in Coventry is an Age independent service, One team with two functions, We operate a 24 hours service and the shift patterns are 11.5 hours. This fast paced, dynamic environment requires excellent problem-solving skills, the ability to make decisions under pressure and provide high quality care to people with mental health illnesses and social inequalities in their own homes. You will also be expected to Triage Urgent referrals which include Safeguarding and make decisions about care and treatment in a responsive manner, signposting to 3rdsector services and referring into secondary care services if needed. Giving current NICE guideline advice around self-help and treatment options via telephone triage. You will be supported by a senior leadership team of experienced nurses and an MDT of colleagues who are passionate about treating patients in their own homes in order to avoid hospital admission. We are based within the Caludon Centre and cover the city of Coventry. Car driver preferred but not essential. We are looking for registered professionals who are compassionate, motivated and approachable. As the successful candidate you must have a professional qualification i.e., nursing, social worker, with substantial post registration experience as a qualified professional of which you will have recent experience working with clients within a secondary care mental health setting. You will be responsible for performing telephonic triages, face-to-face assessments, risk assessments and formulation of crisis care plan/s and discharge preparation for our service users. The team/s operate a 24 hour Rota system and you will be expected to work various shift patterns. Having access to a vehicle is deemed a requirement for this role, for the purpose of community visits, however due to changes in the service, those who cannot drive will be considered for office-based work, performing telephonic triages or face-to-face assessment at base. The MHAH & HTT provide a service to people experiencing an acute psychiatric crisis (functional or organic) who require urgent assessment to determine the required interventions using the least restrictive environment to facilitate recovery. The post holder will provide; • Telephonic triages • Urgent Assessments of mental health needs and risk • Formulation of plans of care in an acute crisis • Medication home treatment • Evidenced based intervention that follow the cluster pathway • Monitoring concordance to care and treatment plans • Support Care packages as required to meet the needs e.g. Organic patients • Contribute to the review of care • Supervise junior staff providing care delivery The post holder will also be required to manage on a rotational basis the operational duty system, and provide a decision making process regarding workload, skill mix and resources. The post holder will be required during this span of duty to ensure that the team deliver the roles and functions as per operational policy • Gate keeping • Facilitate early discharge • Assessment (of mental state and risk) • Improved access for patients out of hours • Home treatment • Support Care packages as required to meet the needs e.g. Organic patients The post holder will ensure that all organisational systems and processes are adhered to. The post holder will manage and organise competing and conflicting priorities and manage change in complex situations. The post holder will be a resource of clinical knowledge and competence in the service area contributing to staff and service development. The post holder will be required to support the organisations agile working policy. The post holder may be required to act up in the absence of the team manager and to undertake delegated responsibilities as required.
